All youth who attend The Sandy Club are required to have their information filled out and signed by a legal guardian or parent before participating in any club activities.

Each individual must provide payment before services can be rendered. This ranges depending on family income but are always kept affordable for a school and summer memberships.

Youth will need to renew their paperwork each school year and each summer (if attending for the summer).

The use the drop-down menu under Sign Up to select “School Year Info” or “Summer Info” below will direct you to the necessary forms. Please print and return to the club. Please call with any questions.
